1.  Origional passport valid for at least 6 months from traveling date (with enough blank pages for visa).

2.  For Non- American citizens, if applicable, clear COLOR copies of your green card – front & back.

3.  4 Recent passport-size photos - White background with your name signature on the back of the photos.

4.  Visa Application please fill-out in INK, one application per person.

5.  Original vaccination certificate for Meningitis vaccine

6.  If you are not carrying a Muslim name, a certificate from the Imam of your mosques indicating that you are a Muslim is required.

7.  Husband and wife holding different last names, a copy of the marriage certificate is required.

For Ladies Traveling for Hajj alone.

7.  Women traveling alone (must be over 45 years) without a mehram, require a NOTARIZED permission letter from the mehram, indicating that he has no objection and permits her to travel for Hajj with Dar El Salam Travel group. Mehram: Father, Husband, Brother, Adolescent son.
